Important. All applications must be filled out in their entirety. Your application is not approved once sent. You will be required to present me with a valid photo ID before I approve your application and book your session.

To speed up the process, you may send me via email a picture of your ID and a selfie or a selfie of you holding your photo ID next to your face. Your photo ID must be legible. You may cover your license number and address. You may not cover your name, photo, and birthdate. The name on your ID must match the name on your application.
